Bookings and queries can be made by contacting our Hawthorn rooms on 03 9819 3499. Naomi and Lucy may briefly discuss the nature of your issue, your referrer details and provide some basic information about your appointment.

Your First Appointment
Please arrive at least 15 minutes before your initial appointment to complete registration paperwork and a brief medical questionnaire. Please download and complete the patient registration form and send to admin@hawthornsportsmedicine.com.au.
Please bring to your appointment:
- Your referral letter, all relevant reports and other correspondence you deem necessary
- List of current prescription medications and supplements
- All relevant medical imaging (X-ray, Ultrasound, CT or MRI scans etc)
- Medicare card
- Details of health practitioners you would like involved in your care (physiotherapist, osteopath, podiatrist etc.)
Please wear clothing that will be convenient and appropriate for examination. We ask that you wear shorts for lower limb injuries and a sleeveless top or T shirt for upper limb injuries.
Payment is required on the day of consultation with part of the consultation fee claimable through medicare. As we are medical specialists, rebates are available through medicare, however our consultations are not eligible for private health rebates. We accept EFTPOS, Visa, Mastercard or cash.
